Please print the New Patient Information form and our Patient Demographics Form, fill them out, and bring them with you for the first visit. If you need help, we can do it with you once you come.

Also, please make a copy of the front and back of your health insurance card; otherwise, we can do it once you are here for the first time.

If you are coming for Acupuncture for the first time, please also complete Measure Yourself Medical Outcome Profile- you will do the shorter version before each acupuncture visit.

Our current Acupuncture Fees for Dr. Moroz are $120 for a 1-hour session, and $60 for a 1/2-hour ear acupuncture session. Acupuncture Extern treatment sessions are provided at no charge once a week.
